Run START triage at a mass-casualty point
Practice the NDMS START order from MCI training. After the walking wounded are directed out, score each remaining casualty with RPM. Treatment beyond the 30-second screen waits until the first pass is done.
- Direct anyone who can walk to a designated green area.
- Move to the first non-walking casualty.
- Check respirations.
- Open the airway once if the person is not breathing.
- Tag black if there is still no breathing.
- Tag red if respirations are over 30 or started only after the airway was opened.
- Check perfusion (radial pulse or capillary refill).
- Tag red if perfusion fails the immediate criteria.
- Check whether the person can follow simple commands.
- Tag red if mental status fails.
- Tag yellow if respirations, perfusion, and mental status are delayed.
- Do not stop for full treatment until the first pass is complete.
