Handle a vaccine cold-chain excursion

Practice the CDC Vaccines for Children cold-chain order from immunization-clinic training. Doses are not administered until the manufacturer or immunization program says they are usable.

  1. Notify the vaccine coordinator or supervisor immediately.
  2. Read the data-logger download; do not discard vaccine yet.
  3. Label the affected vaccines “Do not use” and isolate them in a working unit.
  4. Keep the vaccines at the last known good storage condition.
  5. Document the excursion times, temperatures, and lot numbers.
  6. Contact the manufacturer or immunization program for usability.
  7. Administer or discard only as that guidance directs.
  8. File the excursion report.
