- Confirm the permit covers this space, this work, and this shift.
- Isolate the space from energy and adjoining process lines.
- Station the attendant outside the space.
- Test oxygen from outside with a calibrated direct-reading instrument.
- Test flammable gases and vapors (LEL) next.
- Test toxic air contaminants last.
- Ventilate if any reading is outside acceptable limits.
- Retest until acceptable entry conditions exist.
- Don the required PPE.
- Don the retrieval harness.
- Attach the retrieval line to the mechanical device or fixed point.
- Have the entry supervisor sign the permit to authorize entry.
- Post the signed permit at the entry portal.
- Maintain continuous attendant contact until the last entrant exits.
