1. Write the ordered dose, concentration, and time on the worksheet.
2. Set up the conversion so unused units cancel.
3. Solve for mL/h or drops/min.
4. Compare the result to the usual range for that drug.
5. Set the pump or count only after an independent double-check if the drug is high-alert.
6. Do not ‘round to a nice number’ that changes the dose.
