Complete a head-to-toe physical assessment

Reinforce the nursing head-to-toe order from clinical-skills training. Documentation is not closed until airway/vitals, then systems from head to toe, are finished.

  1. Identify the patient.
  2. Obtain vital signs.
  3. Inspect and assess the head, neck, and airway.
  4. Auscultate heart and lungs before the abdomen if the protocol requires it.
  5. Examine the abdomen.
  6. Examine extremities and skin.
  7. Complete a focused neuro check.
  8. Document abnormal findings before leaving the bedside.
  9. Do not skip to a favorite system first.
